Go to their profile page and click on the "Send PM" button at the top just below their name.
If you know their handle name and it appears you don't, you can always go to "Mail" tab, click on "Compose" and type their handle in the "To" field.

So I did a review search and found a provider I would like to contact however from the review page I don't see any gold envelope to click on. In the past I have just clicked on the envelope from the forum post or the forum ad to PM them but this particular provider does not posts on forums. I know she is active because of the back page ads and personal website. I could email her but normally like to PM so they can see my profile/whitelist referrals easily. Is there anyway to see their actual forum name to PM them? They are usually different from name on profile.

So after your post I went back to try it and discovered there is no send PM button. I then tried a few random profiles and found them on there. So if I don't have the option can I assume she was removed or doesn't have VIP etc?
When a provider has her profile linked to her username she gets free limlited provider VIP. This gives her access to PMs, and will put the "send PM" link on her profile, as well as the "see my Reviews" link under her name if she posts on the board.
