From the "Main Menu", click Escort Reviews and then click Submit A Review.

Two tips - 1) do your review in a word processing program first, and paste it into the appropriate two sections, and 2) make a copy of the provider's profile page so that you can fill in all of the details the first time through (the program will prompt you later for any blanks you didn't fill in.)
