I'm new to this and had a great time my first time out.  Things didn't go so great with the second provider which I blame myself for.  No full service.  My question is this - when the provider goes into the bathroom, am I supposed to strip down then?  Completely naked right away or leave on my skivvies?  Get under the blankets for false modesty or lounge nude on the top of the bed?  Suppose you set up a 2 hour session and the provider comes in and used the bathroom right away; is that too early to get undressed?

The standard practice among most providers on the first visit is they will ask, "would you like to get comfortable",which means disrobe.They like you to undress first,this is one way to let them know you are not LE.I usually strip to my boxers,either lounge on the bed as you put it or I will stand up and gently embrace them.Don't be pushy,just try and let them know your decent and not LE.Sometimes they have asked me to take them off,but usually we just proceed from there.
One reason why I like to see only a few with whom I click well.They will greet you with a warm hug and kiss and are usually dressed in a very sexy and provocative way.

the right signal is when she tells you to "get comfortable". You can leave the skivvies on. Whether you get under the covers or not is your choice. Usually I don't because I want to help her out of her clothes, for me that's a good way to get things going.

I would suggest, taking off your shirt, shoes and socks.

If it is a two hour session, you don't want to start the first minute..pace pace pace.

Most men that I have seen can only release 2 times in 2 hours.

Don't rush yourself..have fun..kiss and make small talk.

I was waiting for a "let's get more comfortable" kind of signal but never got it.  I'm not real comfortable being super pushy so I was still clothed at the end of 2 hours.  This provider has great reviews and was really nice which is why I tend to think this was my fault.  So once again, if she says "I'm going to use the bathroom" like 40 minutes in, is this a signal to me to disrobe?

Unless you're really into talking, and not into the other pleasures she has to offer, 40 minutes into the session and no touching or viewing is a little too long to wait. I enjoy talking with a personable woman, but I also am a man and enjoy visuals more.

It sounds like you were sending signals that you either were making her nervous, or you just wanted to talk.

Then again, I may be wrong.

During the get to know you chat, I sometimes start to touch her gently on her arms or legs.  Normally she takes that as a signal to start the proceedings.  I'm not a really pushy person either but not getting naked after 2 hours is a bit extreme.  You don't have to be an a-hole but sometimes you do need to put out the signals.

Some times I am a little nervous and when I get nervous I ten to converse too much. I have been lucky so far for when this has happened the gentleman I was with simply reached out and touched me. A simple slow stroke, down a thigh, along a arm, does it
for me.  

And as jaejae  says sometimes signals get crossed and it is hard to determine if a charming chap is more visual or tactile.

You should let a lady know if things are not happing the way you wish. She will understand. I would.

Always pick a provider that is well reviewed if you are unsure of how to handle yourself.  You will know the services that they provide, so it won't be as uncomfortable for you the move forward on to the next step.  I generally will not ask a provider if she does this or that or whether it is the right time to make a move.  Do your research and you will know.  Like others have said, they could be waiting for you.
