I'd like to be able to get a credit or debit card that (a) provides statements to me only via email (no paper); (b) that lets me have a card issued to my ATF without having her be anything other than a cardholder -- i.e., she doesn't get involved in my credit rating and I can cancel her at any time.

Any ideas?

Never give a credit card (credit or debit) to an escort, even an ATF. Sorry to so blunt. Sounds like a possible SD relationship. If so, just give her cash at the beginning of every  month.

Get an AMEX issued by AMEX (not third party banks like Citi).  Additional cards have their own unique card number.  Primary account holder can set individual charge limits on each "additional card(s)" and you can revoke individual "additional card(s)" without having to change your primary card number.

I told my SO about this "cool" feature and she gave me a look...
