on her site is there a "contact me" page or form?
i would start off by filling that out with EXACTLY all of the info that she requests.  in addition to that after you are done, i would also send her an email stating that you have filled out her form on her site and if there is any additional info that she needs that you are more than happy to supply it since you are very interested in meeting her
short, simple, respectful and willing/able to give her all of the info that she requests.
then let her get back to you.

First and foremost, honor whatever request she has for the contact medium.  Most providers list somewhere how they would like to be contacted (text, phone call, e-mail).  

Send her the requested message, keep it brief but be honest and, if you truly fit her criteria say so...something like:

"I saw your ad, I'm a 50 year old married guy looking for an occasional tryst.  I understand the information contained in your ad and would like to try setting up an appointment.  I'd like to schedule something for late afternoon on Thursday.  Let me know how you would like to proceed"

In all likelihood she'll get back to you with some questions regarding verification, and possibly even some bio info to make sure you are the stable guy she requests on her site.

Some guys are leery of giving out too much information.

On the "Contact" page of my website, I actually posted a PERFECT introductory letter I received from a guy (obviously didn't post his real info) to help some gents.  It's worked well and surprisingly, some actually READ it! LOL
