As for the link, you just need to be patient now. It takes anywhere from a few minutes to a few days. Depends how busy admin is. "contact us" is probably the best method as sphinx and swim said, but the problem report should also have worked. Email also works ([email protected]). I'm sure you will be linked soon, but if by chance you don't see it after about 48 hours, then shoot them an email, (since you already used both other methods).
First of all, the "special buttons" on provider accounts that we don't see on our accounts are for whitelist referrals, not for linking reviews. Secondly, until they have reviews linked, there is no difference between a provider account and a hobbyist account. So kendall's acvount for now, dies nit distinguish her as a provider. She has no special buttons.
On another subject, it appears you are still moderated. I notice your posts go up late. You should contact admin and ask to be unmoderated. You have enough of a posting history, without breaking rules, so they should unmoderate you.
Contact admin using the contact us button and ask to have review linked to your handle. You can also ask for limited VIP so you can read the juicy details of your own reviews, but not those of others, that you have to pay for.
