Click on "advanced search" and search by name and city, that should do the trick.

I would like to search TER to review a provider or find out if she is reviewed on TER when I don't have her TER id. I would like to just type in her provider name and phone number and hit search, but I haven't found a place on TER to do this. Can it even be done?

I even installed the TER Plug-in. I highlight the provider's phone number, right click and click on Search on TER, but it always says "No records were found" when I know for a fact they are reviewed on TER. Any ideas on what I'm doing wrong?

The TER plug in/ app (or whatever the official name is) is a waste of time and memory.  It's better to highlight the number use command-C (or ctrl-C for the M$ world).  

Make sure you set the city to "all cities" just to make sure she'll come up if she's in the system.
