So I was with a provider recently who I had given a nice gratuity to in advance, and as things began to heat up, she gave me the option of CBJ, or BBBJ for "a tip."  Later, when I asked for something else, she complained that she was used to getting extra tips for "all the extra stuff" I wanted her to do (one example of "extra stuff" was my desire for CG over DS).
I am relatively new to the hobby, so I'm curious: is it common for a provider to ask for additional gratuity for services that other providers include as part of their normal service, or is this a more unique circumstance?

as well it should because, outside of MPs, it has no place anymore in this day and age.

Most respectable gals do not upsell (One exception is Greek, and that should be made clear on her site.), and therefore you should be able to find plenty of gals who will give you their full menu for one fixed rate based on time only.
