1.  The city and state where you are located (You'd be amazed how many gals leave out this little detail.)  If you travel a lot, put in a travel location calendar too.

3.  What screening info you need.

as far as contact info can it be just your email? I usually prefer to give out my number thru email. I think that would be easier then trying to screen thru a bunch of phone calls... ?

Also should there be "standard" information that should be posted...i.e. like all escort sites should say "blah blah blah"

Some girls like phone contact, some prefer email.  If you don't want to put your phone number on your site, you don't have to.  If that's the case, be sure you tell whomever is going to write your first review not to put your number on your profile.  As long as you have one form of contact on your profile, you will be good to go.

As a client, I prefer initial contact by email also, eliminates nervousness of talking to someone you have never met in person.

That is why i like email first, because for some reason I am horrible on the phone. In person its like no biggie...but on the phone im um..ah...oh...dur.... lol!

I always post email first, phone later. If I lose a few then I lose a few but if I dont feel comfortable then i cant make them feel comfortable.

Frankly, I think it's better to make contact over email and then send the phone after the preliminaries are underway.

when convenient for you and you will not have your phone ringing at all hours of the night.  I never understood why some ladies list a phone number in their ads.  IMHO, it would be much better to give out the number once you have screened the client.
