What is the standard for tipping. Ex. If I get a massage with a hj, and the donation is $, is a $40 tip sufficient?

depends on what is going on.  If I do an overnight with dinner, breakfast plus entertainment, then I don't tip for obvious reasons.
For shorter events if they want booze, I bring a big bottle of their favorite (not mine) and let them take the excess home.  I bring flowers and if I want them to model something, I will usually let them keep that too.

Agency girls that I have a good time with I'll tip $50 to $75.

Indy's I tip no more than $50. Although twice I tipped $100. They were very deserving of the extra money.

The above were GFE types. I usually don't like to see a lady unless she is GFE. I like'm sweet. And I like'm dirty between the sheets.

Most ladies that I see have a fee of $300 to $350 per/hr.

For agencies, I am about 20%.  For indies, I usu. don't tip, but I often bring gifts if I know something about their likes.  This topic is a huge area of discussion in various circles.  Lots of opinions.

To me, all inclusive is all inclusive. The advertised donation covers it all.I do, however, bring along some wine and maybe one of the gift items that the girls indicate on their web site.

I always bring flowers and sometimes another gift (body spray, lotion, wine etc.) depending.

With regards to tip, it is very much at your discretion.

For agency ladies I usually tip about 20%, although more sometimes.  For indies it varies, usually not because they don't have to split the money.  It also depends on the size of the indy fee.  I am less likely to tip on an all-inclusive fee of $300-400 then $200.
