Thank god I have never had a problem. I guess one should eat a lot of bran and clean out the system the morning of the appointment. They also make these special shower heads (looks like a stainless steel carrot) for this very purpose. I saw one once hanging in a gay co-workers bathroom. I asked him what the hell is that thing? He explained it was a shower bidet and you can see it in the attached picture.
