SP Posted an Ad on TER (several reviews)
Contacted provider week prior and booked an appt.
Night before appt, I asked she also available that day. SP said yes and confirmed via several email exchanges an appt.
Given location but not addy. Asked for my eta which i provided.
Arrived at location on time and sent email update but then email exchanges stop.  
Sent 2 follow up emails w/ no response
After 30 min waiting around called it a day cut my losses.

Next day get an email saying she thought she "already gave me the info"
Sounded like BS excuse but no harm no foul.  
Gave benefit of doubt and confirmed specific time later on in 2nd night. # and hotel name given.
Arrived at location on time. Called and emailed. No answer.
Becomes a repeat experience of yesterday so I didnt bother waiting 30min
Still no response after i left.  
I write another email saying shes a flaker and that her reviews are fake. I get FU as response.

Did I do something wrong

Most of the time, when a girl is 'mail only' , it means she has a person doing the screening and setting the appointments for her.

The first time you booked, it seems that she was busy with someone else and not able to confirm your appointment but you were given a location by her screener and went there anyway.

Not having a way to have a live communication is a big issue for me. Some gentlemen also try to book just by mail, which for me is too impersonal and bound to create last minute issues.  

I don't mind if we do the booking by mail but, few hours before our appointment, I do like to spend few minutes on the phone with my client so I can give him directions or any instructions he might need. Also, is more personal than "mail only", right?

Next time, make sure to get the girls number before heading to her location. And, if you cannot get her to talk to you prior heading out, just make sure you - indeed - have an appointment set before leaving for her place.

Never see anyone without a tel # provided so that you can stay in touch throughout your ETA.  Any provider that does not provide a tel # and only wants communication via e mail with all the lame excuses AVOID.

you did everything right.  I usually like to talk to a guy before the visit.  This way you get a sense of who is showing up.
