While unlikely, STIs can certainly be transmitted that way. I mean, they can also be transmitted through BBBJ, which most girls do offer. The larger issue with doing this act uncovered, for me, is that it could easily be seen as a teasing invitation for BBFS.

It can definitely put you (in a generic sense) at risk for STI's. Herpes, syphilis, and HPV are three STI's that are transmitted primarily through skin-to-skin (more specifically, genital-to-genital) contact.
In general, condoms are most effective against diseases transmitted via bodily fluids. Not so effective against the three I named above, for the reasons that all skin (shaft of penis, balls, perineum) is not covered (posted a link to a CDC article).
