Is there a time period limitation to put them in? Have a few from May and June I never put in and got a couple of emails from providers questioning why I never submitted one,(one I saw twice) was anything wrong kind of thing?. Is it too late? Normally they don't want anyone to know I saw them so I'm kinda shocked. lol

January, April , August, or whatever. Don't get hung up on the 90 day thing.  If the review is honest, and fair, I wouldn't worry about writing a review that occurred in June for instance. Just date it for September.
