A review that was removed as a fake is probably reported by the provider as such.  If TER believes she wrote it herself, then she would be delisted entirely and her profile would be gone.  When profiles are merged, whichever id number they are merged under should be the only one left out there; the other profie should be deleted.  Of course, mistakes do happen.

Yesterday I saw a new review of someone new to the business, but today it's gone. Any reason that would happen?

It could have been a reported as a false review or self-review. For example if the reviewer is using the same computer as a provider, thats one way that TER admin can determine it. Also sometimes TER doesn't catch review problems with their initial approval and the problem may have been reported later. Providers may also request delisting but that will remove everything and prevent future ones as well.

The reviewer may have asked to edit the review for any number of reasons, in which case the review will vanish until the edited review is submitted and approved. The one time I did that the second approval took much longer than the initial approval.

TER will merge a "new" provider account when a second account for that provider is mistakenly created.  There was at least one instance of that this past week.  There was also a new review that had one name for the provider but a completely different name that was used several times in the description.  I would guess that one has been cleaned up by now.
