I regestered Dirk-Diggler as my alias just because when I was testing different screen names I found it was available, and I use it in a different forum elsewhere on the web, so I wanted to prevent it form being used here by anyone else. I have been a troll under the bridge of TER for years, always taking and never giving back, But I've had a change of heart about this and decided to offer what I have(For what little it may be worth!) Of late, I have started posting reviews, and joining discussion where I think I can be of help. I whole-heartedly agree with you on the points you've made about not having the balls to say what you mean and stand by it. My purpose in asking (while using my alias) was a straight forward question, so thank you for giving it a straight forward answer, and not just flaming me like I'm some fool who doesn't belong here.

If I post with my alias, is my true screen name hidden?
What if I review with it, do still get the VIP credits?
I'm just trying to figure out how having an alias could be useful to me? - any thoughts or creative ideas?

They are for those peeople who wish to make statements and not have the balls to back them up. They are also for making statements without fear of issues being pointed towards them.  They serve a VERY SMALL benefit.

Typically I do NOT like them and hate when they are used.

To answer your question, your "real" screen name is not shown or know while using an alias. Only the moderatoers, staff know the identities.

When you review, you can ONLY review under your MAIN name.

Why bother with an alias? As it is, you already have your PM name as it.
If you don't have your name and your word, you don't have squat.           SP00

Aliases allow you to post without fear of retaliation from providers, who, for example, may not appreciate your thoughts on current prices or customer-service practices. While the use of aliases has been abused in the past, there is no question that anonymity has its value at times.
