4) Shower or hot towels - it is nice to have the option. This is were an apartment would work well.
5) Sheets or towels to lay on - definitely sheets
6) Lady's clothing style - I am a sucker for topless with a pair of sexy panties.

- DEPENDS ON THE GAL
Massage table, futon or bed: It’s a mood thing pretty much. If I’m going to an office setting, I’d expect a table, but, being the klutz I am (yes, I have fallen off a table a time or two), the futon is usually preferred.
Office setting or apartment: I think storefront would be my first choice, office the second. The hairs on the back of my neck usually stand up when I go to apartments or homes.
Lotion, cream or oil: Bare, untreated hands first, unscented oil a close second. Lotions and creams just feel too slimey for me, especially if a shower isn’t available.
Shower or hot towels: Showers, hands down.
Sheets or towels to lay on: Sheets; they always feel cooler and more comfortable.
Lady's clothing style: For a strictly therapuetic massage, it doesn’t matter, but if a place tries to upsell with a lingerie option, I know they have no confidence in the massage skills and I won’t either. For extra play, however comfortable she’s willing to get is good by me.
Email or phone call for contacting: It doesn’t matter a whole lot, although a voice can give me a better handle on the lady and her personality.
Location: Burbs or Metro: As long as it’s within driving distance, easy to find, and has decent parking, it doesn’t matter.
Therapuetuc w/big tip or Sensual at set rate - I've seen a lot of 411s for therapuetic ads: Therapuetic is what I look for because I really do like to get those knots taken out.
If reviewed, LOTS of reviews or a few will do: Several reviews are good for me, especially if they’re from multiple sources (TER, EroticMP, and so on). It’s always nice to get a concensus.
Length of Massage: 30, 60 or 90 for first time: 60 or 90, depending on the rates.
A must in ad for you to want to contact i.e photo, rates, website or link to reviews: Green flags for me are all of the above, along with a list of modalities and even a friendly paragraph by the therapist. Red flags are ads written in text-speak, filled with typos and extraneous characters, and a belligerent attitude. And I hate to sound biased in any way, but photos can make or break a deal, too.
Massage style: therapuetic with light touch or all sensual/erotic: Therapuetic to start, hands down. If the comfort level and the satisfaction are there, I’ll repeat and I won’t be adverse to other (non-painful) styles.
Topless or Nude, do you expect to pay extra: Depending on the price advertised. If the therapist is advertising $60/hour, I’d expect extra for those options. If she’s advertising $150/hour, I’d expect those options to be built into the price.

Prefer last min scheduling or a days notice: Unless I’m making plans for a trip, I’d have to hope for last-minute availability.
If emailing, how long do you wait for a reply before moving on: I usually don’t wait. When I’m making those plans for a trip, I usually send out a few emails, just feelers, and hope to hear from someone. Usually, I’ll get just one or two replies so then it becomes a matter of 1) who has the friendliest (and most professional) text voice, 2) who gives me the best vibes, and 3) who was first.
If no pics, do you ask over the phone for stats or wait til you show up: Hopefully there’ll be something in the reviews about this, but, no, I don’t ask. It’s not something I think of. I have been…misled before, so I have to rely on any vibes I get from her voice and our conversation.

Massage table or futon -
Choice is good. Heated is nice.
The option for a futon on the floor is nice (most beds would feel awkward for therapeutic massage).
A sturdy massage table can work well too, because it can be easier on the provider's back, etc.; a comfortable provider can mean a more enjoyable massage.
Office setting or apartment -
The following types of issues are more important than whether it is an office or apartment. Privacy: not a lot of other people coming and going at or near the entrance, secure, quiet, excellent soundproofing, lack of nosy neighbors etc. Not cramped.
Lotion, cream or oil -
oil (the glide seems best), always unscented. Natural, not mineral oil or baby oil (yuck!). It seems that a blend of oil and unscented lotion might also be fine.
Shower or hot towels -
Both!
There are some times when it is needed to have the shower available first, and most times it's good to have after.
Hot towels when you're wiping off excess oil from the back before the flip and hot towels are nice for cleanup (warm wash cloth is good too) even if taking a shower after.
Sheets or towels to lay on -
Either or both is fine as long as they are soft, comfortable, and laundered and dried using unscented methods.
Lady's clothing style -
Discretion is good. "Normal" attire. Don't make the neighbors suspicious. Dressing as most therapeutic-only massage providers would is fine. Casual is fine, some cleavage is nice. During the massage, the fewer clothes the better, ease of removal and dressing again may influence the above clothing choices.
Email or phone call for contacting -
Both. Email only for INITIAL contact is fine for me, it is discreet and I do understand that ladies have to put up with a lot of "rude stupid calls" when a phone number is published. It surprises me how often I've met a lady in person and had her tell me "You were so nice on the phone" when I thought I had merely been polite and discrete.
There are times when email is not convenient and other times when phone is not convenient. Some ladies will have a phone number for ads that only goes to voice mail (to weed out the jerks), and another more "private" number for clients they are willing to see.
Location: Burbs or Metro -
I prefer Metro, first-ring suburbs, close-in suburbs, in that order. I prefer to avoid what appear to be "massage-unfriendly" municipalities. Things like parking, privacy, crime rate are more important than whether it's city or suburb. There was a lady that had an excellent apartment incall in a nice location that I saw several times. Unfortunately, parking was so bad that I would have been walking a quarter-mile or more in the winter to the closest available parking spot.
Therapeutic w/big tip or Sensual at set rate -
Definitely Sensual at set rate. Having to discuss the "tip" feels like a bad idea for multiple reasons.

I do prefer to see a photo (it's understandable if you don't show your face), but if the ad is intriguing enough for me to call I might still make an appointment if the vibe on the phone/email is good. Published rates are a must. These don't have to be in the ad itself, as long as they are on a site that you link to. For me, you don't need to link to reviews as long as they will show up when I google the phone number or email address. I'm also more likely to make contact if there is some indication of location in the ad. For example, I believe your location could be described as "convenient SW suburb". An indication in the ad that the provider is independent is also helpful.
Massage style: therapeutic with light touch or all sensual/erotic -
Undraped therapeutic transitioning to erotic. I usually also enjoy getting at least some therapeutic attention on my front half after the flip. Sometimes, on a subsequent visit there may be a preference for "all sensual/erotic"

It is nice when the provider gives an indication of what her normal expected email response time is on her web site or ad. Ladies' communication preferences vary widely; if she has a preference for phone or email I want to know. It makes things easier for both of us. If I was emailing in the morning for an appointment for the next day, I would move on if I hadn't heard anything by that same night (unless I know that she only checks it once a day, for example, but then I probably would have called or emailed further ahead).

No, I would not ask, that would feel rude to me.

30+, with more "+" not being an issue so far. I'm not sure how much this preference is related to my age or how much it is related to the feeling that experience counts. You can often tell that someone has been performing massage for several years when you see her arm muscles.
