does it bother you guys when a girl excuses herself and comes back with a cover that has been taken out of the package?    Just for some reason that bugs me.

It was probably a just mistake from hurrying.  

If you're in a situation where you may get caught with covers when they aren't used at home & prefer they provide them but if it really bothers you that much and you're worried it could happen again,
 then next time you should just get a pack & leave the remaining ones with whomever you're with.
