I'm planning my next hobby experience, and am wondering how to best include a kink or two.  I would like to have the provider snowball after an oral pop, and I would also like to wear a butt plug during the session.

Another idea I had:  Would providers object to a prospective date sending a fictionalized account of en erotic endeavor?  Obviously, no contact or appt. details would be mixed in with this message, but it would give the provider an idea of what I am looking for.  She could respond back either favorably, or she could reply that the scenario described wasn't her cup of tea.  Has anyone done this?

I don't think it would be all difficult to find someone into your fetishes. In fact, I am sure you will get some responses here on TER from providers who can accomodate your requests. But, of course, it will be easier (and more sanitary) if you bring your own equipment.  

To get some of my own fetishes fulfilled, I often pick out a provider who I "guess" might be agreeable, book a short first session, just to get through the safety part,  and then bring up the ideas after we are more comfortable with each other.   Often she will be agreeable, and then I will book a longer session later.  

I would have to guess that any provider who allows BBBJTCIM is pretty open minded, and can provide just about anything else you might want.

Those two things you mentioned are on the milder side of kink so I think you shouldn't have any problem finding a willing  provider
