How many people actually do get gifts for their providers?

A couple of times and subjective.  A couple of ladies have been extremely generous IMHO.  The next visit I've repaid with a gift.  Just my way of saying you've done something others havn't, I recognize it and appreciate it.

The last one I gave as a b-day present. I've given plenty away as x-mas gifts. And others I've given just for fun!

I brought a special lady I've been seeing a sexy new outfit this week. As I told her.... damn it looked good!

Once I get to know a lady (3 sessions in or so), I get to know what she likes.  My ATF has gotten tips, home cooked meals, wine, beer, a pack of gum, and a new thong from me.  Currently, because I know green is her favorite color, and lions are her favorite animal, I'm trying to find a green lion for her.  However, I know I'm in trouble when I can't find an exotic item like that at MOA...might have to go back to the drawing board for the next session.

Sex toys doesn't count. I have had alot of that. I give them out or eventually trash them.

Of course they are new and in the package. Gifting a used sex toy would be disgusting. Also I tend to be a little leery about the ones that get pulled out of the nightstand.
I'm ok with sticking a new toy in my mouth. The one from the nightstand? Not so much.
 

There's a few who are my ATF's that I've known for many years, it's also nice to let a person know they're thought about too so I try to return the favor

I find that regulars are more likely to show appreciation this way. Sometimes for a special occasion (anniversary, bday, winter hols) but occasionally there's the "just because" gift which is such a sweet and unexpected gesture. Still, I never, ever expect them, so any time someone is kind enough to do so I am deeply flattered and grateful!
