Actually I saw her many years ago and-was an excellent provider. Has delisted so am wondering why TER has allowed this thread to continue as guidelines prohibit discussion of delisted providers.

Is it a provider choice or TER admin decision?

In K's case it was primarily about a fake review and the board comments that followed. Can't remember the specifics, but she and her friends were injured and offended and have been turning their noses up at TER members ever since, unless you can change their minds.

It can be either.  TER can delist anyone for sufficient violation of TER rules.  Providers can request to be delisted although last I heard they had to show they had gone UTR.
