For mine its auto deleted and I wont see it
if it's not completed properly for these reasons :
No phone number or bad phone number
No name.
Bad email address.

Could also be because she hasn't gotten to yours yet.
I get anywhere from 20 to 300 emails a day, that's a lot to sift through.

Have contacted a provider regarding an appointment.  On her website she states to use the contact form.  I used the form, double-checked, and sent it on Tuesday.  How long should I wait to try again or just move on.  Have not received a reply from her as of today.  Thank You

Try again now but work on a back-up plan. She either has more emails than she cares to deal with or she's on vacation or something. Don't obsess over her as there are always choices.

an email to the provider simply letting her know that I had filled out the form and was hoping to hear back.  Just that.  Not a lot of the same info in the email.
I give that a few days at least before moving on, especially if it is someone you truly hope to see.

I did that on Wednesday.  I know she is very low volume.  My instincts are to give her the weekend.  In my situation, unfortunately cannot reveal, I don't have alot of choices.  If I were to give her the weekend, would that be sufficient enough time?  Thank you all, again

You can followup with her again at that point and/or start looking elsewhere.

There are a number of reasons I have never used these web-site forms for requesting an appt.  First, they are unreliable depending on how the web-site is hosted.  The end result is generally, but not always, an email that goes to someone with that information.  As a principle i don't like any personal information being sent to somewhere or someone that I am not aware of.  Second, these forms almost always ask for personal information and that in itself is a non-starter for me.  Many of the providers who ask that an appt be requested this way will accept other ways of being contacted such as email or by phone.    Many are also listed on verification sites like P411 and Datecheck and those services have the means for requesting an appt with any of the provider members and generally the provider is going to prefer that anyway because it helps identify you has a member.  

However, if you have used the form and have also emailed her, then she has probably taken some time off and is simply not monitoring her email.  You may or may not hear from her at some point.  But one repeat request in about a week would be a good idea and if still no answer just move on.  There could be something in the info you provided that has spooked her.  A lot of providers will simply ignore an email from someone they are suspicious of rather than get into any correspondence exchange.

and I would never use them for that reason. I've gone through P411, and if that's not enough for safety, there will be no connection. I don't mind giving more personal information about my habits, etc., though, and that's easily done via email.

I would never fill out one of those forms.I'm looking to have some fun, not find a job.I'd move on to someone less formal.
