I hear ya... it seems like everything's going OK here in MN 'til you start traveling, and then you realize just how different things are.  In Denver, no-call no-shows, either by hobbyist or provider, are virtually non-existent...  It has become a common problem for both here, and in Denver it just so unacceptable that no one dares do it, it's just not accepted like it is here.  In Denver I've:
-Never once been stood up.
-Never once been upcharged.
-Never once been bait/switched, asked for tip, or asked for rvw.
-Never once had an appointment where MSOG was not just available, but was assertively offered, often demanded.  
-Never once paid more than 2.5 for an hour (plus, see below).
-Never once had trouble getting a reference.
-Never once got hassled back when I used to write reviews.
-Never once had a provider decline to see me because of reviews, or other gals I'd seen they didn't like.

And as you said, Rise, the appointments there are simply different.  In my experience, MSOG has been the norm, the appts. tend to go just a tad over an hour, usually about 1:15 or so, and if a gal is feeling perticularly chatty, she'll make it a point to tell you "don't worry, the clock hasn't started yet."

It's just different, hard to explain, it's just completely different.
