Waiver form

Anyone ever heard of a provider requiring a new client to sign a waiver form that they are not LE before she will see them?

I have heard of agencies doing that. You sign their waiver and pay xxx$ to talk.
I haven't heard of indie providers doing that, but more then likely the scenario will be the same.

You should NEVER sign anything in the hobby. No good can come of it.
