I've been to many that offer back walking and are very skilled at it, especially Chinese IMO. I've had it probably a handful of times but really prefer the hands and forearms. Not personally big on the elbows and knees or the backwalking.
Also, many many ladies are excellent at "therapeutic" massage as they've had hundreds or even thousands of hours of training and are licensed. I've even had a few ladies who were medical doctors back home doing massage here for various reasons and they definitely know the body. I would certainly research your particular injury and discuss it with your doctor but I'd have no trouble getting therapeutic massages at an AMP.
Re the HJ and further extras, it's definitely YMMV not only dependent on the place but also the location in the country. Where I currently am, extras beyond the $40 HJ are almost nonexistent at the $60-70/hr AMPs. However, at some other places in town they are provided but those places start at around $140-$200/hr and then negotiate and upsell everything from there.
One last point for the OP, I'd advise you to enjoy a TS (table shower, not the other) as often as you can. Good experience, fun, and YMMV something may happen.

See a licensed massage therapist for that, and if the condition is really bad, an orthopedist.
As for what goes on, it varies a lot. I have never been to an AMP that does the walking, and would be a bit concerned to let someone do that unless I knew they were very skilled at it.
What normally goes on is that you get a pretty good massage and for about $50 more a HJ. If you're lucky, you'll find someone willing to go further.
