How do I report a hobbyist. Please help

Hi,  
I need help!! I'd like to report a horrible hobbyist who has been nothing but trouble and ridiculously flaky. He's try to reschedule 3x, never wound up seeing him due to the 4th request. I have a cancellation fee which he completely went MIA.  

Warning to all providers, he's a shady flaky person!

Please help me in reporting or blacklisting

There was a guy who booked with me so many times I lost count . He showed up once .  It was so infuriating that I called his work (  a medical or dental office ) . and . I  booked a  bunch of  fake appointments to teach him a lesson !   He went too far with it !
