It depends most on who you're talking about.  If it's someone with lots of reviews, she's not likely to call the office to ask if you're a good lay.

Without the benefit of knowing that she's reliable, I'd be a bit more tentative.  The bottom line is that she wants what she wants, and you're willing to give what you're willing to give.  If they're far apart, maybe she's not the girl for you.

I'm considering filling out a form for a provider here in Vegas, but the form's details are, well... detailed.  I believe she requests FULL name, Business name, and business phone #.

Is this normal?  Is it mandatory that I fill out every question?  Am I worrying too much?  I'd rather not have a provider calling my office because nothing good can happen when a provider is calling your work.

I understand and agree...  My recent "shopping" for a provider was downright shocking - some of the webforms out there are more detailed than credit card applications.  Thankfully there are still a few independent providers that ask the right number of questions and/or do appropriate reference checking to make sure they will be safe, but don't make a hobbyist feel like the CIA is checking into their history.

If you have my name, email, work phone (which I answer), I have posted reviews (of people that can be checked with), I can be googled AND I carry a driver's license (to fly of course) that matches all the info, that's about as out there as any hobbyist should have to be.  

Or put another way, if you want a trusting relationship, don't ask me for anything you're not willing to tell me about yourself.

While I understand the need for some privacy, you do have to look at the situation from their side:

1) Are you LE?
2) Are you safe?

There's a reason some provider ask for detailed info because they want to have some measure of comfort in seeing you.  It helps them relax and provide you with the great service that you deserve.

On the flip side, there are horror stories about providers that have abused this privileged info.  Few and far between but I understand where you're coming from.
