It's not easy, but not hard either. You have to take the whole areola into your mouth, as opposed to just the nipple. It's a slow, full suck that gets the milk out
Whether or not a woman knows depends on how heavily she's lactating. When you hear them swallow, you know. LOL If a woman is nursing a baby exclusively, sucking her breast will cause milk to let down, even from the breast that isn't being sucked.
As for me, someone would have to suck harder than would be comfortable to get any milk outta me. The only way I can get it out these days it to s-q-u-e-e-z-e my boob. Am no longer nursing any offspring, so the flow has decreased considerably
FYI, most American moms don't breastfeed, so it's a narrow group of ladies here who would have personal experience.

I love your nipple-to-areola ratio, I really do! But they (the areolas) are so small (read perfect), I'm guessing sucking just your nipple would be kinda hard.
It's all pretty interesting to me. I don't think any of the girls I've ever been with have been moms.
