Was just totally stood up by an apparently very well reviewed provider.  Went through the ringer on verification despite my TER profile and then she simply falls off the planet - no emails, texts, phone.  A bit concerned about the verification info.  Any advice?

You're an experienced hobbyist, so you know the drill. Nothing you can do about verification info, btw, what did you give her. If you're not already a member join P411 or Date Check. If you're still in Vegas, try the 2 legit agencies, HaleyHestonPC or 100ProofOfLV, lots to choose from with either agency.

That you were stood-up.Maybe something unexpectedly came up on her end or maybe she just decided not to see you there is really no way of knowing.You said you are concerned about verification info there is nothing you can do about already giving a provider your info.Was an appointment actually confirmed after you provided this info?
I would suggest moving on there is nothing else you can do.

She contacted me early this morning and said she simply fell asleep. She seems legitimately apologetic and I believe her, despite my dissapointment.  I guess shit happens.

You should let the rest of us hobbyists who we should stay away from.  Most of us only have a few nights to have fun so being stood up is a real bummer.

She overslept? and you really believe that? and did she offer to make it up to you. Vegas is full of great providers,  but these things happen, a whale came in town and booked her for an overnight, or a great client called at the last minute and wanted to see her.  There are a ton of reasons why she did what she did. Just remember a plan B is a good idea in this town. Contact the agency that responded on this thread, get pre screened and you won't be in this predicament the next time you come to town.
