I’m aware all of the hotels have restaurants, there’s yelp, etc. but I also know that when I lived in Vegas many years ago (eay before the hobby for me), my favorite Italian place was something no one would have known is there. So who knows some hidden gems?

I don't know how locked in you are to the North end of the strip, but If it's Italian you want, check out Noras.

Otherwise I had a great evening at  
Sushi House Goyeman,  its kind of a dive and not a place to go for a nice night out,  but the food is definitely good.

If you like authentic Mexican food try Lindo Michuacan on Desert Inn just east of Eastern. It's about 7 minutes east of the Strip. Food is excellent.

If it's not doing enough business, it's going to close and get replaced, so you can't really have something hidden.

That being said, I think Noodle Asia on the casino floor is under-appreciated because it's not where the other restaurants are.  Luke's Lobster outside Fashion Show Mall is another place that often doesn't seem as busy as it deserves to be.

Lindo Michochan and Nora's both are great local choices.
I also liked the Legends of oyster bar and grill

Take a short Uber ride to the Arts District (its between the Strip and Fremont St). There are some pretty great places to eat there. Esther's Kitchen, Main Street Provisions, Good Pie (for pizza), Soulbelly (for BBQ), and great craft cocktails at Velveteen Rabbit, or great micro brews and a very small but delicious menu of bar food at ABLE Baker Brewery, to name a few spots in the Arts District on Main Street.

Even closer to the North end of the Strip, but a few blocks east on Sahara is one of the best steak houses in town, The Golden Steer. Lots of old Las Vegas history in it too. If you want to stay on the Strip, a great steakhouse on the north end is the Steakhouse at Circus Circus. The casino is a dump. The steakhouse in it is a hidden gem.

Tons of great eateries can also be found west of the Strip on Spring Mountain. Sparrow & Wolf, Kabuto, Raku, Edo Tapas, are a few of my favorites there.
