A provider in my area (DC) is asking their clients to subscribe to the "RSVP Project" which supposedly is a national (or at least regional) service to ensure you are who you say you are.  In addition to a nominal cost, you are asked to provide your name, email adddress, place of employment, and cell number.  Additionally, it asks for your date of birth, last four of your social, and your mother's maiden name.  In return you get a member number and PIN which can be used to make appointments without hassle.  I'm all for ensuring the safety of everyone involved but providing the last three bits of info concerns me.  Anyone have any experience with this?  Thanks!
