It's simple. If you're unsure, simply ask the provider if she would like you to provide a review. You could even ask as a further gesture of respect if there is anything from your time together that she prefers you not share.
You can do your job and be respectful and encourage others to be the same. However you can't protect them from the next guy that may not be as considerate as you are. Their job is to protect themselves and stay safe by screening to whatever degree they're comfortable.

I've had some conversations with non-vip providers who've been on the receiving end of some heinous treatment. I've also seen some of that behavior reflected in less severe terms in the content of some reviews. I am of the opinion that we have a humane responsibility to limit exposure to providers who may be at risk in one way or another of receiving poor treatment, e.g. New providers who operate out of hotels they are occupying full time, etc. I'm new to this and so there may be some unspoken etiquette around the idea already. Flame on.

First, you cannot really protect a provider by not reviewing her. Leave out details that you do not think should be shared, sure, but you flatter yourself if you think the review can do much more than either slightly help or hinder her business. If she does not have the sense to take care of herself, there is nothing you can do for her unless she asks, and that would open up a whole other set of questions and issues.
Second, as posted above, I always ask for permission before reviewing.
