I heard a story a few years back, not sure who told me, oh well. But this provider gets a call for a client she had not seen in a while. She tells him to get comfortable but he doesn't take off his t-shirt. When she questions him he tells her it is just to cold in the room. So they get more comfortable, she hears a loud snap and her client starts to scream. Turns out the client had had back surgery but thought the provider would not see him so he didn't tell her. They called the clients brother who came over and helped get the guy to the hospital.
