Had two such experiences. One with a provider whose oral hygiene was deplorable to say the least. I avoided all oral contact, simply buried my head, got my jollies and left. Me being me, I would probably offer more of an incentive $ if she originally declines the shower since I've waited and anticipated the appointment this much.
If that didn't work I suppose I'd just finish and chalk it up to 'unrealistic expectations': move on write a 'less than stellar review' and eat my losses, but then again, it all is determined about the 'chemistry' (I hate that word) at the time.
I have experienced this before. I simply said, "Baby, I am sorry. I don't know if you are aware, but you have a strong odor that is affecting my interaction with you. I want this to be enjoyable for the both of us. Can we take a break and freshen up?" Yeah, awkward but no way could I drop the coin I was dropping and deal with that smell. She cleaned up, hit the mouthwash, apologized and we were good to go.
