Most times I've used ladies request forms, I got no feedback whether the form was sent. If the lady put the same required information into a document which could be copied into an e-mail, the onus of completing all the blanks would be on the gent. And, he'd be able to readily keep a copy of the information given out ... which would be good for me, a least.

I got this idea from a provider, who has since changed her website.

That is a great idea!  I will do that as soon as my new website is up - hopefully in a few weeks (a little late for this tour I know).  I have grown very tired of mistakes like this being made myself!!!
