I agree on a 90 minute appointment, especially if it is your first visit with the provider. I know some gentleman probably think ladies push the 90 minute appointments solely for our financial gain but there is a method to the madness. It sounds like the original poster's concept of GFE is not solely the acts, but the experience. More time leads to better rapport, in my opinion.
As for the issue of expecting the provider to stay....yes, I have stayed over the appointment time before. Typically I will do this if the gentleman and I having a particularly interesting conversation, if he is a very nervous newbie who needs extra time to feel comfortable, or he's a regular to whom I want to give a little extra attention.
