I think any need for a shower is within the 60 minute time frame. It's not like you need to take a 20 min shower when you walk in the door, or before your ready to leave.
I think most women give themselves time between appointment to freshen up. Having some knucklehead singing opera in the shower for half an hour after the appointment is the last thing the women whats when she knows some else is due.
Hopefully most guys is smart enough to take a shower right before they get there. (if they can).
Remember Time is Money. That applies to both parties involved. 
i agree with portdog. i paid for an hour to be with you, and when i'm with you that's your time. and if showers are involved, then i want you involved, too.

The shower is part of the hour. But it does not have to be merely hygiene. It can be...Brooke, can you wash me again down there? And rinse again down there? And...whoops I left some suds on your left boob!
