People take technology for granted....kind of like indoor plumbing.
Because it's been around a while, and we use it every day, we don't really give it a second thought and just assume it will always work flawlessly.
Unfortunately the real world is different and things don't go exactly as planned. So sometimes you end up wading in shit.

Booking forms don't always work, and neither does email for that matter. I've had 3 occasions where I filled out the booking form and heard nothing back. Followed up days or weeks later with a polite email. All three times the reaction was surprise that the booking form didn't forward the info via email....and an apology from the lady. Wound up meeting all three ladies (not at one time...lol) and had a great time. One, I've been seeing for 2.5 years and we still joke that had I not sent that email and just blew her off.....well....I never would have gotten blown. LMAO

Saw a lady was visiting this week that I have been curious about so I decided to book using the on-line form on her website.  To be honest, with all my years of doing this, it's the first time I've ever actually used one of these forms. It appeared to be her preferred means of contact so I went for it. I never heard back, not a word. Now, clearly she could have just been too busy to see me. It's also possible that something about my contact info sent up a red flag-doubtful since I've never been refused an appointment by any indie or agency and my reference is recent and very good.

I was going to try email or PM but decided to simply skip it as I was seeing my ATF this weekend anyway.

I'm curious as to what kind of success rate others have had with the "submit your request" type of bookings. It only took a minute to fill out the form but it also entailed contacting another lady and ask her to be ready to provide a reference. Ad to that the fact that I submitted personal information in what appears to be a wasted effort.  

Please, no posts about "moving on". I already did that. Just looking to read about the experiences of others.

I sometimes have trouble with my booking form.  I answer all emails, as I am sure most ladies do.  So I believe that you should have given her another chance and wrote her directly to her email.  The internet is not fool proof.

I would tend to agree with you. If I didn't have a fall-back I may have tried email.  As I said, her site indicates that filling out the form is essential so I followed her wishes.  I'm not trying to blame the lady, just looking for other's experiences.

This is pretty much what I suspected. The next time the lady comes to town I will most likely take a different approach.  Thanks for your response DD.

Yeah it's happened to me where a potential client sent me his info 3 times in all before I actually got ONE of the submissions (never got the first 2)....so I think if one has a booking form she should also allow email JIC let's say the gent sent a submission and she doesn't reply in about 24hrs

Unlike a direct email (gmail, yahoo, etc.), there are too many extra eyes that can view an on-line form submission. It's being processed by her web hosting ISP which can be a huge corporation or some local guy with a server in his basement ... or some foreign guy with a server in  his basement in China or Russia.
