when I request a date?

There is nothing more annoying than "u available?" be it text OR email.  Even worse is the "hey baby, cum c me" type.  Different ladies have different preferred methods of contact and typically they will state as much on their ad or website.  As a general rule any method of contact should include (in my opinion) the following basics:

1. Introduce yourself.  "Hello, my name is ..."
2. State clearly what you want. "I am hoping to schedule an appointment for date/time"
3.  Thank her in advance for contacting you in return  "Thanks, and I look forward to hearing from you"

Personally I always give priority to those who include their screening information up front as well.  It shows me they are serious in their desire to meet.

It is also a good idea to include a little info about yourself in your introduction, just a few sentances. Some ladies even specify on their website: "tell me a little about yourself." It helps set you apart from the many other emails she receives and further shows her that you are serious. JMHO

That way I have everything needed for screening, and don't have to spend time going back and forth to get the required information. It really makes everything much easier for all involved.

I agree. The form is the most convenient way and saves a lot of time, going back and forth can be mind boggling. I've learned that some gentlemen dislike "submitting" forms so I ask them to copy & paste the form into an email and complete it that way.
I like to have my act together so being organized is important to me. The screening form is absolutely the best way to make initial contact.

I don't like the forms . Who does that form go through to get to you ? I prefer the 'going back and forth ' , personal emails .

Generally speaking, most web forms simply grab the information, send it in an email to the provider (or their screener), and don't save it anywhere.  
That being said, it's entirely possible for that form to email to anyone, to save in a file/database/whatever on that server, some other server, etc.

But the same thing is true for "personal" email. If Google wanted/cared to, they could read all your personal email on gmail. Same thing for any other web-based email provider.  

Text messages are marginally more secure, however, a telco can snoop on those just as well.  

At the end of the day - you have no privacy at all if anyone really wants to know what you're up to. Fortunately, most people just aren't that interesting. :)
