You need to start with looking up kinky GFE providers, because GFE is code for a lot of the things you mention wanting in a domination setting.
As you know, some kinky GFE providers aren't dominant but rather submissive, and some aren't all that kinky and just use it as a marketing word.
But once you visit a website and read a few reviews, you should have an easy time discerning who might be super kinky and dominant and who isn't.

It seems like you've learned the basic lessons already- that it's difficult to find someone in your area who is in effect a "crossover" Domme, somebody experienced and equipped in the BDSM world AND willing to venture into GFE activities.
There are many women whose websites advertise both services, so look for travelers, check the reviews here, and look for GFE providers who say they are fetish friendly and ask about that service.
I would say it's probably easier to find a GFE who has BDSM experience, than it is to look for a Domme who offers GFE service.
Finally, you might be able to move further along with a Domme after you have seen her several times. Oral pleasure might not be offered on the 1st visit, but after you've served her a few times, it might be possible.

Traditionally, a provider who is strictly a pro domme oftentimes doesnt allow for intimate contact. However, there are those of us (myself, Tori, and many others included), who combine intimacy with kink, fetish, and bdsm seamlessly.

Another tip is to say "I'm used to seeing escorts" That way, the domme sort of gets the CLUE you're seeking more of a kinky gfe experience. References and screening information offered up front help as well.
