...had some or all of the providers you have seen or tried to see not come through with flying colors in the area of customer service and public relations. You would think that with that kind of compensation the five things you list would be easy to take care of. But, none of us knows what the rest of us deal with on a daily basis, so I'll draw no conclusions. I will say that I don't see or won't continue to see providers that allow those things to happen on a regular basis (note I said regular basis because we all know shit happens) and it interfere with my schedule with them.

1. I answer my emails pretty regularly.  However, I do not answer my phone as regularly as some providers.  This is because one of the perks of this job is having a lot of time for a personal life, and I am out living it.  If I'm available to pick up, I will.  If not, feel free to email or leave a message.  I'll get back to you.  
2. My car is always running.  I agree with you here.  Priority.  If something went wrong, I would have it fixed.
3. I own a watch.  Don't wear it.  I'm pretty prompt in spite of that.
4. I can get up whenever the hell I want.  I happen to be an early riser.  However, I am sure there are some of you who would sleep much later if given the option.  
5. Aside from once, I always show up.  I'm not sure of the ratio of providers who actually show up to those who rarely do, but with the exception of an emergency, I am one of the former.

I'm saying if I made 300 an hour, I would show up everytime.....I'd answer my emails and voice mails,  I'd be on time and I could keep my car running ( thus the excuse, I couldn't make it because of my vehicle would NEVER have to be used).

Of course this is just hypothetical.

You seem to expect some high degree of appreciation for that high rate.  You fail to realize that once that rate is accepted by the marketplace, it's no longer high.  If my attorney is booked solid at $250 an hour, he can blow off the asshole client knowing full well that another will fill the spot.  I'd love to fill my day at $250/hour customers, so that seems like bad logic.  I can assure you that people annoy me daily and regardless of the rate, they always will.  How's that for biting on your topic in the name of drama?

There's just really no meaningful way to respond to it.  "Oh yea, I don't like people who charge me money and then give me bad service either."  Now what?  Was that profound and thought-provoking?  Is that some exceptional statement that runs counter the sentiment of everyone else in this hobby?  Do most people like it when they're blown off or made to wait for a scheduled appointment?  Just doesn't really qualify as stirring things up.

that your basic assumption of $$$/hour applying to an 8-hour work day or 40-hour week is just not realistic.  Yes, you (or me) pay $$$ for an hour with a lady; can't really multiply that by 8 for a day or 40 for a week because few if any providers see clients back-to-back-to-back all day long.  So even if a $$$/hour provider sees 3 clients/day they're grossing a bit over $ on an 8-hour day.  And yes if they did that 5 days a week 52 wks/year that'd be a tidy sum.  But they most likely spend the rest of their week answering queries, contacting other ladies for references, reserving hotels, buying supplies, and working out to stay in the great physical shape we all love.  So it's more like a full time job for which they occasionally get a nice fee for an hour and much of the time are working to set up more appointments & manage their business.  And who are we to say that's not honest work?

So while I may agree with you that timeliness, keeping appointments, being responsive to e-mails/phone calls etc. are what we ought to expect from a provider (that's just being professional), you shouldn't think that the hourly rate you're paying for one hour of her week gives you some sort of speciall status or a set of high-sounding rights.  She's surely spent time setting up the appointment, maybe getting a room, preparing the room, preparing herself, etc.

So when stuff happens, I'm much more inclined to just say 'Rats, too bad' than to denigrate the provider.  Maybe I'd never try to see her again, but I wouldn't necessarily act like some sort of high-roller willing to spend $$$/hour and thereby expecting/demanding unfailing  commitment to 'my hour',
