Being truthful with your doctor is extremely important but keep in mind that what you tell him could end up in your medical records (a forever document) and impact you later when getting medical insurance or life insurance.  Talk to your doctor ahead of time about what he'll put in your record and what you want him to know.

For fear that something might be put in your forever medical record, you may wish to admit that you were involved in an erotic escapade that got out of hand but not that it was with a provider.

If you are not truly single, you'll need to make sure that the Dr. has no connection to your SO so as to make sure the beans don't get spilled somehow.  The one thing you probably don't want to admit is that you were hurt during a hobby experience.

It may be important to admit what physically happened to cause your injury but it is none of the Dr's business to know who you were playing with and under what circumstances the 'relationship' occurred.
