I have done it to a local provider (more than once..) - which last time ended up in a classic full vigorous fist fucking that looked like something out of a porno.  My fist has an 11 inch circumference which is about 3.5 inches or so in diameter - which is a lot smaller than a baby's head.. though still pretty impressive....

It takes time, gentleness and persistence and, obviously, some relaxation, trust and willingness on her part - and it might not be possible even if she's willing to give it a shot.   Definitely a YMMV sort of thing. My guess would be that if she  has not had children, it might not be possible and even if she has had children it might not.  But, like you say.... they can sure stretch..

Work up to it gradually.  This is something that you would do as part of an all afternoon session not a one or two hour gig.  Use a latex surgical glove.  Start with some small dildos, work up to the larger ones - 2", 2-1/2" diameter. Use a bullet vibe on the clit.  Three fingers then four.  Be VERY slow and gentle especially as you tuck the thumb inside the fingers for the initial hand entry.  You'll know when a fist is possible - don't force it.  And, of course, lube, and lots of it.  Begin to slowly move the fist forward and back.  Read her signals to figure out when to start giving it a little more action.  Don't force it - you don't have to - all you have to do is stay firm.  If she's really into it - she will actually fuck your fist.  When it started getting vigorous, switched to a Hitachi to put her over the edge into a screaming O....
