I haven't seen many providers in a couple of years but I know referrals are needed by providers.
What is the best way to handle this?  I know some providers won't remember me after a few years.

The only reviews that you are showing that aren't active is Sarla-What would you have to lose?  So suggest you email those gals and see what happens-you may even want to reconnect with one or more of them to get restarted....
