OK, I will assume most providers do not use real names for obvious reasons. As clients, who uses their real name? Who chooses to not use any name?  
If you use a hobby name, have you ever been asked for photo ID?  Did you show it or refuse?  
If the names did not match, did you have to explain.

There are different ways at looking at this. The laid back providers who do little or no screening will just ask for your name , whether it be the name you go by or your full name. Then when you arrive, some will ask for some form of Identification (Most prefer drivers license) and some won't. The girls who follow that process do this because they have been around long enough to tell whether you're LE or not.  
Then, the girls who do require your full name mainly ask for screening purposes. They'll ask you for your full name when you send them a date request. The date request can be from a verification site (I.E. Date-Check, P411, etc...), a personal email where your provide additional screening info besides your name (phone number, references, board handles, etc...), or most ladies have a request form right on their website where you provide the same info you would in an email. I assume providers can see our real names when they check our profiles on verification sites, which can help them faster with verification .  
So far I've never seen a provider where you disclose your hobby name unless you inform them that you will be reviewing them under that name.  
And when providers ask for a photo, some will just ask for a picture of your face, and some may ask for a full body photo (Dressed, not buck naked). This gives them a better Idea of who they can be dealing with. I had a provider ask me for a face photo because she usually does not see men of my age range (20's). I texted her a photo of me, and she gave the go ahead for a date.

I was asked to show an ID the first time a met a provider but I didn't have any references at the time.  She did screen me but I think this was an extra level of security/comfort for her.  It didn't bother me though.  I was asked to provide a photo as well to a different provider, those have been the only times.

I do give them my name, since I am sure they are going to figure it out anyway when they screen.  I do not use a hobby phone but probably should.  I think it may be harder to explain the extra phone though.

I think having a hobby phone is a good thing to have, as I known some providers that carry two phones in order to keep things separate or use a virtual phone number like google voice. If you already have references lined up, then you are going to have to notify them of a number change before you can use them as a reference.
