You can't review a visit that never occured. However you are free to "name and shame" her on her regional board!
I know it's irritating, but you have to enter all the details every time you submit a review. Your best option is to open he existing profile in a new tab, or new instance of your browser, and copy and paste.

I recently booked an appointment with a provider who subsequently asked me to reschedule several times to the point I eventually just gave up and canceled. I'd like to share this experience so others are aware but submitting a review requires that you provide details of what she looked like, etc when I didn't actually meet her. Should I submit with the answers from other reviews and just explain in the text or is their another way?
Also, is there a way to submit a review for an existing provider without having to re-enter all of the data?
