        The answer –health care and medical- 11 in one year. That is IN ONE YEAR Dude. H’mm.  Want to add them all up and run that percentage by sector? I didn’t think so.

         You also conveniently forget that I offered that metric only in response to Jack’s “proof” that Medicare is projected to go insolvent in the future. This “opened the door” to a comparison of private companies, whether privately or publicly held, that go insolvent. Public companies represent a fraction of the total number of private companies and they still are regular visitors to Chapter 11.

No amount of bureaucracy can fix a system if those issues are not remedied, and those issues are remedied by Congress spending the needed resources to do so, having bureaucrats in the VA who are competent and care about the institution, and getting rid of anyone in the system benefiting from activities of corruption.

More funding would be beneficial but it would also result in more waste, graft and corruption. It's administered by the federal government, after all. In most cases the private sector, because it has to be accountable to customers ,does things better than DC. So when Mari posts "that's not what the law states", it doesn't matter, they'll ignore the law, re-write it arbitrarily and change it unilaterally to suit their political needs at the time but at the same time, it will place HC and the solvency of the program in complete ruin.

         The Dept of VA is the second largest agency in the federal government with over 300,000 employees. This agency provides direct health care services to vets along with other services. The VA typically has 8 million or so enrolled patients. That is a massive undertaking to manage.

        Once it is up and running, I doubt more than a 1000 employees will operate the  non-tax provisions of the Affordable Care Act.  For all the talk about the ACA being socialized medicine, the ACA leaves the provision of health care services to the private market of patients, insurers, and doctors, subject to ACA regulation. In fact, once the regs are drafted, the ACA will be largely self-executing with the IRS doing most of the heavy lifting at tax time.

        And before you give us the “private sector always does a better job,” how many  health care companies have over 300,000 employees or 8 million patients? Answer- none. The best private counterpart to the VA is the HMO, once billed as the solution to rising health care costs. Guess what – they are going the way of the dinosaur- they did not really work that well even with private management, despite being a fraction of the VA size.

 
        So the VA mess is in large part due to its sprawling, unmanageable size. Of course, there are going to be bad apples in there doing a bad job unknown to the top brass.

          How many private sector companies are poorly managed? Have you driven your Chevrolet lately? How are your accounts at Bear Stearns and Lehman Brothers doing?  Check with Arthur Anderson if you have any accounting questions.

        When poor management is uncovered, size is usually the prime suspect.  

One day we are going to see some intelligent criticism of the ACA on this Board – but not today LOL

I have no clue as to why you have so much faith in this horrendous law or this incredibly inept WH.  

Please point out to me things large (or small for that matter), the current federal government does well. Size is irrelavent when incompetency reigns.  

There was a much better way to handle the HC problem we have and you know that Mari. It should have been a hybrid between the Obama goodie bag and free market ideas never tried.

There were many great ideas that for political reasons, Obama didn't want to accept. Serious tort reform was one of those ideas that had wide bi-partisan support and would have really helped with bending the cost curve. Buying across state lines was another. Either one a magic bullet? No, but they should have been just two weapons in a full arsenal to stem the tide of ever rising costs.

All the things you have mentioned in the past as benefits (staying on your parents policy unitl 26, getting rid of pre-existing conditions, non-discrimination based on age, etc etc etc) put upward pressure on costs. There was never any serious attempt by O-care to address cost and cost is just one of many things that will force its demise.

       Sorry, the lame Obamacare is “a bunch  of lies” whinning is not substantive criticism, particularly since to date no one has able to actually identify a single lie as opposed to negligent misrepresentations. The “Obamacare will someday have “death panels” is equally off base bc a true “death panel” would be unconstitutional and if this happens “some day” in the future it would be the work of a different president and Congress.

          Most of the criticism I’ve seen comes from posters who do not really know how the law operates.

       As it happens, I agree with you that the law would be improved if it provided better cost containment provisions, although that discussion would lead us to the Independent Payment Advisory Board. (See link). Whether tort reform should have been included, or defensive medicine costs otherwise addressed, however, is a very complex question which we’ll have to save for another day

As for the "lies" that isn't really worth arguing about. You will make a technical defense of what constitutes a lie or not but most people trust their common sense, experience and judgement and have determined Obama lied to the American public. It has been widely reported that Obama's policy team differed from his political team back when they were selling O-Care. Barry chose to side with the political arm knowing full well many would lose their doctor, but it would have never passed w/o promising the "keep your doc" b.s.  

With regards to the criticism by posters you refer to, I dont believe anyone really knows how the law operates. It is a changing dynamic. People on the left just a few short months ago would have said people can keep their docs and a family would save an average of $2500 under that ACA but we now know both are untrue for many. The law "operates" differently now then what was sold to us, then what was promised.

        You are now in charge of  a vast network of filthy, dilapidated hospitals and thousands of doctors who finished in the bottom half of their med school class. Vets regularly complain about the quality of care they receive.

        One day you read in the newspaper “George Bush invades Afghanistan.”  A few months later you read “George Bush invades Iraq.”

        You immediately realize that your facilities –which already are struggling to provide quality health care - are soon to be flooded with thousands of new combat veterans.

         What are you going to do?  You can’t charge these vets market rates and tell them to get health insurance. Combat vets by law get absolutely free health care for 5 years and thereafter get mostly free benefits for service connected injuries, although there are co-pays and maybe deductibles – not sure exactly how it works.

         Congress increases your budget a little bit but not nearly enough to pay for better doctors, better facilities, and to provide this medical care.

        So now I’m truly fascinated. Tell how me Jack Dunphy, private citizen, can manage this mess better than federal employees.

        Still sure that federal employment is a “significant commonality?”

Thousands and thousands of businesses are well oiled machines day in and day out. The problem is that virtually ALL of big government is broke and ineffective.  

Even Obama himself has admitted the private sector does things better, especially with regards to IT HC. (See link)

The rules should be relaxed to allow out of network options during a crunch of patients like we saw after Iraq and Afghnaisitan. That is how a corporation would handle this problem better. It can snap its finger and allocate resources differently. Why? because it HAS to. it has to answer to its customer base, its stockholders, its employees. Big government simply has no structure or incentive to do anything fast and efficient.

And to address your last paragraph, once again, it was the FEDS that allowed it to grow in massive size without proper oversight. Yes, Mari, this really is the problem of the feds.

          That idea would just multiply the cost of providing veterans with health care tremendously. If a vet could choose to go the shiny new private hospital down the street, or a state of the art private practice with those open MRIs, that is great for the vet and I agree it would sure take care of the wait times problem.  

        But wait a minute. Now we have a stampede of vets using the private option. But Jack Dunphy CEO still has to pay the 300,000 VA doctors and other employees whether they work or not.  So the government ends up reimbursing the private doctors while still paying the VA doctors.

Iit's harder than you think.

       As to the 10 to 1 bankruptcy point, I think that is actually true – the number of private businesses that fail is astronomical. It is not just that SOME fail -the majority do.  Sixty percent of new businesses fail, according to the SBA. Add the existing businesses that fail within 5 or 10 years and the number is huge.

        When a government entity fails, the cliché lovers come out and say it is bc the government can’t do as a good a job as the private sector. But when you compare  private sector failures to government failures, the facts just don’t bear that out. Bad management is simply no more a condition of federal employment than it is of private emplolyment

The vast majority of bankruptcies are in those that are just starting up, during their first year or so. How many of these entities were just bad ideas or run by poor managers? These bankruptcies serve the very useful Darwinian purpose to weed out defective DNA that does not deserve to become a viable organism. Bankruptcy ratios have to be one of the worst metrics available for evaluating the efficiency of free enterprise.

If you insist in pursuing this metric I suggest you look at those in established companies. How about companies that were viable enough to eventually “go public”? I think you’ll find that instead of a 90% rate it will only be 1% to 2% per year for publicly traded corporations

Much needed layoffs would get rid of the bottom dwellers and cancers currently flooding the VA and killing it to the core. Waste/fraud/abuse wouldn't be just a slogan politicians use to get elected and never see it thru to fruition but it would be a reality in a private sector setting.

That would create a staff that would be held accountable on a day in and day out basis and allow me to accrue tremendous savings so CEO JD could move asssets around to help out on a temporary rise in patients from recent wars and NOT busting the budget.

Obama is slowly acknowledging that the private sector does things better than government. He has reaized that the government wheel of change is such a slow moving sphere as to be unable to tell it is even moving. A CEO would go into the VA and start cleaning house on DAY ONE. Not 5, 10 or 20 YEARS later and have to hear about the scandal only from the media. Obama is realizing all of this. It's about time you did too. LOL

it restricts the pass or fail results to a very short time period. Short indeed relative to the decades it took the VA (or any other government program) to reach their juggernaut size.

Additionally it is unrealistic in the time he allowed to take the corrective actions required.

The VA does most of it's actual medical service delivery quite well. There are some specific services that it does much better than any present private provider can presently provide as efficiently, e.g. PTSD treatment and limb amputation rehab.

the bottom feeders and the poor performers like you can in a private corporation. Just as Congress requires you to give vets 5 years of free medical care, Congress requires you to abide by civil service rules.  

        You have to follow the elaborate procedures for civil service employee termination contained in the Merit Systems Protection Board regs. You can “downsize” but even there you can’t just consider performance ratings- length of service can sometimes trump mediocre performance.

        And here is something else you did not know- the VA is statutorily obligated to give preference to hiring veterans. And when you attempt to downsize vets, their preference rights are a factor you have to consider. Bottom line is it is almost possible to downsize a long term vet with mediocre performance ratings.

         You can terminate for really poor performance but it has to be carefully documented over a period of time. And there are the appeal rights. The employee can appeal to the MSPB and ultimately to a court. It takes years.  

        Indeed Congress is considering legislation to make it easier to fire senior employees but the rank and file VA employees are going to be there for a long time.

         By the time you get rid of enough of those bottom feeders and cancers to make a difference, the Board will have already fired you.

        Hence, even if we turn the management of the VA over to a private company, the managers would face the same crippling structural problems that the federal managers face- the vast size of the agency, the huge number of patients, a budget provided by Congress, and crippling employee security rules.

       Could we just privatize the VA workforce as well to get over these problems? Yes, but your funding still comes from Congress and you can't make a profit giving free health care, so you would never be able to pay market wages to your employes. So that wouldn't work either.  A private company would have the same problems.

Once again, it is the massive, inept and wasteful FEDERAL government (which happens to INCLUDE congress, LOL) that is the problem here.  

We keep removing big government in our arguments to him to fix the problem, Mari keeps sticking big government back in and then says "see, your fixes wont work because of X, Y and Z" not realizing for one second that the "X,Y and Z" is caused by big government! LOL

You discount the ability of private enterprise to administer veteran healthcare based simply on the tunnel vision of how would they transition the existing federally administered system into something else. Your restrictions do not recognize reality.  You mandate the new “owners” begin with a system that has been run into the ground by the former ones. Then you tie their hands for an undetermined amount of time until they can get Congress to appropriately modify existing laws regarding financial aspects.

Where’s your criticism of the existing VA system that puts up with the deceit and probable crimes most recently exposed? The last time we saw something of the same magnitude it happened in an entity called Enron I think. Where are they now? Your other fake comparison about bankruptcies doesn’t even deserve a response. You know very well, given the huge numbers of private companies vs. one federal system makes the number comparison meaningless. To paraphrase your list, where's "Bear Stearns, Lehman Brothers and Arthur Anderson"? All put to death or allowed to die and replaced with ones that work. Can't happen in government.

 The biggest advantage private enterprise has over any federal endeavor is their ability to get rid of waste. The most successful companies, on a regular or annual basis trim 10% of their lowest performing assets (people and otherwise), replacing them ones willing or able to perform. This is very difficult and sometimes virtually impossible in almost all federal sectors. This is why the government has gotten so huge. Even the military struggles with it. From what I know of the VA system, the huge problems are NOT with the majority of providers of service (nurses, doctors, technicians). The problem is their managers. The good ones are prevented from managing the bad providers and the bad ones are now showing up on the 6 o’clock news.  

And oh, BTW, regarding one of your earlier posts. The ACA IS indeed bringing back the HMO. There are many new systems now set up that are HMOs where there is no choice for out of network provider service. The second generation HMO, called PPO (that has been the standard in the private sector) allows for out of network coverage with more out of pocket costs.

The VA is the ultimate HMO, ultimate from the standpoint that they are a monopoly. With the ACA you still have a choice of 2 or more HMOs, at least for now.

But Jacks error was in assuming that federal managers are inherently incompetent and private sector managers inherently competent. Neither is true. The va problems are structural and are not a function of the federal status of the managers. Private managers dealt the same hand could not do any better. But let's give jack credit for trying.

The 10 to 1 private sector challenge is valid given jacks offer of the failure of a single program as proof of the inherent incomptence. He did not realize that private sector success is the exception not the rule.

And the private sector has competition up the wazoo and if it fails, it goes bye bye. The feds just jack up taxes or put it on the Chinese credit card. Christ, under this admin, you dont even lose your job! LOL

And you understood that.

          The answer is obviously “no.” Nonetheless, charging up the hill, you effectively answered “yes” and offered several ideas how you would change tings if you were in charge, including your now famous “I’d clean house on Day One" post.

        I patiently explained whey you could not summarily fire civil service employees like that.

        This seemed to discourage you as we heard no more about your plan “to clean house.”  In your final post, you tried to change the hypothetical - you said you would remove the fed gov including Congress from the equation so you would not be bound by the existing laws, and opted apparently for 100% privation. Smugly, you declared “victory,” or perhaps we should say “mission accomplished” at this point, at this point.

        But, unless your plan is to  start charging the vets for their health care, removing government funding means you have no way to pay for your privatization. And if your plan was to keep government funding but privatize everything else, your plan would still not work bc the gov funding is not enough to pay private rates to doctors or purchase new facilities. The most you could do would be – gasp – to put the vets in Obamacare.

     So, as I’m sure you now ruefully appreciate, the private sector could NOT do a better job in operating the VA.  And the reason why is that the private sector at its best is usually only superior to government when the private sector can operate for a profit.  When the private sector provides an entitlement program such as the VA, there is no reason to believe it would do any better than federal managers.

What most here most likely are not familiar with, is that VA medical care is means tested. e.g. In November, 2000, I was diagnosed with prostate cancer. A tumor was felt on the prostate by the Aviation Medical Examiner during a routine flight physical. Yeah, that's right, the old finger wave trick. The AME was curious as to why I would develop prostate cancer at a relatively young age,(57). I told him, during my second tour in Viet Nam, I often  was responsible for the maintenance on the C-123K, Ranch Hand aircraft. (Those are the ones that sprayed Agent Orange). There were days when my fatigues would be soaking wet from the chemicals dripping from the spray nozzles. He said that prostate cancer was one of the diseases on the VA's list that were suspected to be caused by Agent Orange. Went to the Philly VA hospital to enroll in the VA system for treatment. Had another exam and biopsy. Yep, it was there, positive results. Asked what were the treatment options? The VA doctor gave me some literature to read, and explained the options. Asked when I would start treatment? He said I would be notified.

In June, 2001, after many letters to the VA, I was notified that my income was too high to be treated by the VA. I exceeded their monetary threshold for treatment in their system. Called a urologist, and was given an appointment for the following week. He recommended an oncologist that specialized in prostate cancer treatment. Second week in July, I had brachytherapy, (seed implants), which was successful.

Returned to the Philly VA in Jan, 2002, (first available appointment), to ask if I could get follow up care after the radiation treatment. Answer was again, no. Income was too high. Asked if I should apply for disability rating as a result of the cancer. The VA said I would be notified if I qualified for a rating.

Forward to October 2010. Notified by the VA that I would be given a disability rating of 30%, for the cancer side effects, and for the hearing loss due to noise exposure while on active duty. All that means is that 30% of my military pension is non-taxed by the federal government. Took them 8 years to make that decision. I know others that have waited as long as that.

My neighbor, a Marine with two tours in Viet Nam suffers from PTSD, severe PTSD. He is eligible for VA care, and goes to the Philly VA Hospital. Took him 4 years to get the diagnosis, and another two before he could get therapy. Which basically is....here's 5 different medications. Go home, take them daily, and let us know how you feel.

and he has been dragged down much the same road. I doubt precious few know about these issues. I'd like to think there would be a much more collective outrage if it were better known.
