It means you see the photo of a beautiful women in the ad but when you open the door, you see a 300 old hag.  

Car dealership, retail stores use it all the time, put an ad out promising a cheap price on something, but when you get there,
the only ones available are way more costly.

What ever it takes to get you in the door, that the idea.

What does it mean when a website posts an ad for a provider, labeled  "real with #selfie" and showing 6+ photos of a beautiful woman and then includes 1 photo of a different woman -much less attractive - and calls it "real with #selfie" ?

You could provide the link to the ad.  

Professional glamour / boudoir photos are often modified using PhotoShop to improve the lady's looks. Sometimes, it is so severe that there is very little resemblance between the lady in the photo and the lady that you meet in person.  

Although selfies, pictures usually taken by herself using her cell phone, can also be PSed, the general assumption is that the PSing is minor or zero.  

Think of the regular Arts and Entertainment sites that show a picture of a celebrity after 6 hours in a makeup chair next to a papparazi picture without makeup.  

Does she have a Profile and reviews? What do they say her pictures (Real: y/n; Accurate: y/n) and her appearance?

it's a scam and you'll pay for things that sound too good to be true until you decide to only meet providers who have reviews.
