If she is well reviewed, I probably would not be too concerned, but if she is brand new, or has just a few reviews, there's a real possibility she is an LE decoy, since LE doesn't act too picky about whom they want to see.

Suppose the provider just asks for first name, email, phone number, and day and time of request.  Should that make you suspicious? I mean, I don't like difficult or revealing personal verification procedures, but if she doesn't for any verification (like a p411 account name or the name of another provider that can verify you) is that worrisome or just lazy on her part

she can find out a lot about you. I would not presume that she is not screening you "at all".

have you done your research on her?  However, it's surprising how much information can be found with just a small amount to start with.  Providers have a myriad of ways of checking up on potential clients, many of which we will never know.  Go with your gut feeling, if you have any doubts, don't see her.  Better to be safe than sorry.  If you feel comfortable after doing your research, then do go see her.
I'd be leery if she has only one or two reviews, esp if the reviewers have only a similar number to their credit.
