shadows cast on bed between her arm and body don't make sense

...seem to be coming from two different directions.  So it certainly looks like some kind of photoshop job to me.

At least one light source sort of above the photographer's head.  I think another muted one off to the photographer's side.

Also, VERY severe anorexia.
